I am writing to you today with an urgent request for Baby of Asma Begum (newborn male), UHID NO: 2502012030, was admitted to the Neonatal Intensive Care Unit of Sri Venkateswara Children and Multi Specialty Hospital, Vijayawada.

The baby was born extremely preterm at 28 weeks, weighing 1.450 kilograms, and was diagnosed with several conditions, including:

• Extreme prematurity (28 weeks)
• Respiratory distress syndrome (RDS)
• Screen-positive sepsis
• E. coli-positive urinary tract infection (UTI)
• Pneumonia
• Hemodynamically significant patent ductus arteriosus (HS-PDA)
• Congestive heart failure
• Status post-PDA ligation
• Shock
• Anemia of prematurity
• Neonatal hyperbilirubinemia

Given the baby's extreme prematurity, very low birth weight, and the aforementioned conditions, the baby may require an additional 10 to 15 days of hospitalization. The estimated cost for this extended care is approximately 7-8 LAKHS INR.
